  • Crediting/retaking a course that has already been taken during a previous degree/program or has been part of a non-degree program from any U.P. Campus is prohibited.

Applications for revision in the plan of course work should be made BEFORE courses to be added/deleted/modified are registered. If you are replacing a course simply delete that course and add another. When deleting a course make sure to indicate the originaly approved type and number of units; its only when adding a course that you can propose a new type and number of units. If you are changing the type or number of units simply delete that course and add again with the proposed type and number of units. Courses that have been graded already cannot be deleted. Upgrading of non credited courses (other courses that has been taken and/or graded already) to a credit course (major, minor or cognate) is highly discouraged.

    The PDF form that will be generated should be printed on an A4 sized white bond paper. This form uses barcodes that needs a clean and crisp print, laser print is highly encouraged. If an inkjet is used, set the print quality to high and make sure there is no banding or streaking on the barcodes. Forms with erasures will be automatically rejected. Do not submit more than 1 original copy. Additional copy will be automatically disposed.

    After printing the form 1) sign it, indicate the date 2) have it signed by your approved committee chair and members, indicating all the dates 3) the unit head, indicate the date 4) submit to the graduate school for evaluation and approval of the GS Secretary and the GS Dean.

    THE GRADUATE SCHOOL HAS THE RIGHT TO REJECT FORMS THAT DOES NOT CONFORM TO THE STANDARD A4 SIZE, PRINT QUALITY AND INCOMPLETE INFORMATION (SIGNATURES AND DATES).

    Article 6. Changes in the official program for the degree must be approved by the Dean upon recommendation of the unit head in consultation with the chair and members of the student’s guidance committee. Application for revisions in the plan of course work should be made before courses to be deleted /added/replaced are registered.
